If you are an existing patient, we will provide you with a passcode when you schedule your exam. Please use this passcode to access your information to update and modify. If you are a new patient, please click on the new patient link to fill out your forms.


Please call our office at 323-653-4078 if you are unable to fill out the online form. You may also complete the paper forms appropriate to your case and BRING THEM WITH YOU AT THE TIME OF YOUR EXAM.

What to bring with you at the time of the exam:

  • Glasses/ sunglasses you routinely use
  • Boxes of contact lenses if not from us
  • Medications List (RX / OTC)
  • Medical Insurance card and vision plan information
  • State or federal government issued PICTURE ID card- we are required by FEDERAL LAW to verify identity to bill any medical insurance or vision plans.

Payment Options

Dr. Newman Optometry and Vision Therapy of Beverly Hills believes in providing the best value for you and your family. We have products and pricing that aims to accommodate all budgets and our office participates in most insurance programs. For patients who have little or no vision insurance coverage, flexible payment programs may be arranged. To learn more about our payment options, please call our office directly.

Our office accepts cash, check, and major debit and credit cards including MasterCard and Visa, as well as Care Credit.


Do you have a flexible spending account or health savings account through your employer’s health insurance plan? You can use money in your FSA and HSA to pay for an extra pair of glasses, sunglasses, a year’s supply of contact lenses, vision therapy or Lasik. The funds are pre-tax so your money goes further. The funds in FSA and HSA may have a deadline and expire. Ask us how to use it before you lose it.

Vision Insurance Benefits

Many vision plans have benefits that may expire annually. This means if you don’t “use it,” you “lose it“ until the next year. Since you are contributing hard-earned money toward your vision coverage, there’s really no excuse to skip the annual eye exam. You can always consider using your vision plan toward the second pair of eyeglasses, prescription sunglasses, or eyewear that’s specifically designed to fit your lifestyle.

Care Credit

The Care Credit card is available to help people pay for health care expenses, often with an introductory period that is interest-free. We accept Care Credit for all of our services. To learn more or apply for the card, visit Care Credit here.
